Dyspnea — Definition
Dyspnea is a subjective experience of breathing discomfort or an unpleasant awareness of breathing, characterized by qualitatively distinct sensations that vary in intensity.
Ortner syndrome (Cardiovocal syndrome)
Ortner syndrome is hoarseness of voice due to left recurrent laryngeal nerve palsy caused by cardiovascular disease, classically left atrial enlargement from mitral stenosis.
Mechanism:
Cardiovascular enlargement/compression → left recurrent laryngeal nerve compression → unilateral vocal cord palsy → hoarseness.
Causes
Classically:
Mitral stenosis with left atrial enlargement
Pulmonary hypertension
Aortic arch aneurysm
Aortic dissection
Left atrial/ventricular enlargement
Pulmonary artery aneurysm or dilatation
Why left side?
The left recurrent laryngeal nerve loops under the aortic arch, near the ligamentum arteriosum, making it vulnerable to compression by enlarged cardiovascular structures.
